A Caste Certificate (Jaati Praman Patra) is the Uttar Pradesh government's record of the reservation category you belong to: SC, ST or OBC. It is what residents of Siddharthnagar are asked for when they claim reservation in admissions, government recruitment and promotion. The name, the father's name and the address have to read identically on every document you upload. A mismatch between the Aadhaar spelling and the ration card sends more applications back from the Siddharthnagar Tehsil than any other single reason.
Whatever address the form carries is the one the Lekhpal verifies in Siddharthnagar. If your documents show two, file with the one your residence proof carries and leave the other out.

The most common rejection
Your name must be spelt identically across every document. A mismatch between the Aadhaar spelling and the ration card is the single most frequent reason an application comes back from the Siddharthnagar Tehsil.
