Which address to use

Use the UP address where a family member can receive the visit and confirm the particulars, and make sure they know an application is pending. Applying against a locked house, or a property the family no longer occupies, produces a negative report and a closed file, and no amount of documentation fixes that afterwards.

Keep the mobile number reachable. The number on the form receives the OTP, the queries and the visit call. If you are outside India, use a number that a family member at the address can answer, and note it down. You cannot change it mid-application.

Documents from another state

Address proof from where you now live does not help a UP domicile or income application; the documents have to relate to the UP address. A caste certificate issued by another state is not transferable to UP reservation either. That needs a fresh UP certificate on the strength of family records here.

Collecting the certificate

Signed certificates are downloaded from the portal, so there is nothing to collect in person. Keep the PDF rather than only a printout: institutions increasingly verify the digital copy against the QR code on it.
